All required spaces must be filled in, otherwise the form will not be sent

Port Charges, Taxes, and Government Fees
$261.94 per passenger for 1st and 2nd passengers, including fuel surcharge (subject to change)
$257.92 for 3rd and 4th passengers, including fuel surcharge (subject to change)

Insurance Selection (learn more)

If you select Travel Guard, additional information and forms will be sent to you. These forms must be filled out and returned within two weeks to receive the extra benefits.

If you selected Carnival Protection Plan, the Invoice price will be updated and sent to you.

Deposit
A non-refundable deposit of $25.00 per person is due at time of booking. You will be asked to pay this amount by major credit card upon submitting this form, or will have the option to mail a check to the Iowa EMS Association following completion of the booking.

Due to the constraints of the cruise line, airlines, hotels, and other vendors, we are unable to offer a Pre-Package at this time. We will be putting together various packages to offer to all Iowa EMS Cruise participants. The cost, location, and additional information are forthcoming. Pre-cruise packages should be available by summer 2008. Information will be provided on-line and included in the EMS Cruiser's Newsletter.
