Keith Wesley called the meeting to order at 10:20 AM.
1.0. Introductions:
2.0. Approval of September 25 meeting minutes.
Minutes of September 25, 2002 meeting were reviewed with no changes. Action – Motion by Suzanne Martens to approve the minutes. Second by Tom Brazelton. Minutes unanimously approved.
3.0 Old Business
3.1 EMS Report Card – Mike Curtis reported on the latest draft. Administrative/systems and clinical components were identified and it was thought that PAC should develop the clinical piece and have the EMS Board develop the administrative/systems piece. The clinical part will be redistributed to members for comments, be edited and sent to the EMS Section by January 1st. The goal is to complete the clinical component at the April PAC meeting.
Mike Curtis is still looking for input on the content. Suggestions included: add pain management, DNR, field termination of care, restraint policy, treatment of a minor, treat and release & other medical provider on-scene. The instructions will be developed after content is completed.
3.2 Pain Management – Suzanne Martens reported that the protocol has been completed, but not implemented yet at her hospital. The pain scale and dosage recommendations were distributed for review. There was discussion about treating abdominal pain and whether it should an indicator or contraindicator for pain medication because of possible surgical treatment. The comment was that the template was a suggested guideline and was flexible so it could be modified to fit a specific area or hospital protocols. A discussion of pediatric changes occurred and the template will be modified accordingly. Recommended that IV pain medication require direct medical contact prior to administration.
Action: Suzanne will edit for final review at the next meeting.
3.3 Medical Director’s Course – Keith Wesley reported that all the narratives are done and Rick Barney and Keith Wesley will review narratives for any possible errors. Keith Wesley is adding audiovisual enhancements for the final product. The CD-ROM should be ready by February.
4.0 New Business
4.1 Investigative (Due Process) Protocol – Drs. Wesley and Barney and Nan will review and have draft for next meeting.
4.2 CQI Templates – Rick Barney provided information on the CQI template developed by the CQI ad-hoc committee of the EMS Board. PAC members were asked to get any comments on the document to Rick Barney by Jan 1st. There were some comments on the document, particularly whether the flow or order of treatment was a sample. The critical actions were noted, but order was not key in assessing or completing CQI review. The other issue was whether some CQI cases might be missed because they are identified by the EMTs and may miss patients that should be counted, but are “misdiagnosed” by the EMTs. Action: Rick Barney will convey information back to the committee.
4.3 CPAP Templates – Mark Westfall reviewed CPAP for EMT-B materials and changes were discussed. Dr. Brazelton presented pediatric CPAP for EMT-basics. The suggestion is to conduct a pilot study in the out-of-hospital setting. EMSC will work with the EMS Section to develop a research study. Action: Dr. Westfall will make discussed changes and forward to committee members and Nan.

5.0 State Medical Director’s Report
5.1 First Responder Rule – Rick Barney
5.2 Bioterroism – Rick Barney
EMS is included in Phase 2 of Smallpox inoculation. Discussion resulted in the following recommendation: Recommend that all EMS personnel be educated concerning smallpox and vaccination and that they begin thinking about their choice to receive the vaccination or not. Action: The Department will prepare and distribute an educational packet to all EMS services in Wisconsin.
5.3 Trauma – Dr. Brazelton offered to forward articles to membership. RTACs are being formed throughout the state. Funding continues to be an issue.
5.4 Pediatric component to IV Tech – an ad hoc committee has been established to add pediatrics to the current IV Tech course, which has no pediatric content. PAC has recommended that IO be added and incorporated into the pediatric module. This module will also be available to ’85 EMT-Intermediates to increase their scope of practice to include pediatric IVs and medications. ‘85 EMT-Intermediates can utilize these skills only after appropriate training has been completed and verification sent to the Department. Motion by Rick Barney to approve the above. Second by Tom Brazelton. Action: Unanimously approved.
6.0 Pediatric Items
6.1 Insulin pump education – for both children and adults. EMTs should know how to discontinue the insulin pump (either turning off or pulling out). Motion by Tom Brazelton and second by Rick Barney that this should be added to the curriculums as the education committee reviews content of each curriculum. Dr. Barney should also include this information in his Medical Director’s letter to services. Action: Unanimously approved.
6.2 Protocol to require glucagon prior to IV attempts – Same as above
6.3 BELSS overview – presentation given by Mary Jean Erschen, EMSC Coordinator – BEMSIP.
